Failure Analysis

After extensive analysis it was determined that the thermal liner was cut ~ .125in short. This allowed the closure rings to bottom out without sufficiently compressing the o-rings. The lack of sealing lead to gas blowby and a thermal meltdown of the forward closure. The forward insulating disk was also identified as a risk due to it's one-off construction method, and low quality material selection. (Don't dremel flight critical components from fire resistant bathroom tiles)
